A contract summary template is only useful if it is the same for every agreement, because the whole point is being able to compare two contracts without reading them. That means resisting the urge to add fields for the one agreement that needs them. The nine fields, and why each is there
Counterparty, because most questions start with who. What it covers, in one line, because the agreement title rarely says. Effective date and end date, because everything is counted from them. Renewal term and notice days, because those two produce the decision date. Annual value, because it ranks the estate by what matters. The internal owner, because a contract without one is nobody's. And a link to the executed file, because a summary that cannot produce the document is only half an answer.
Obligations underneath, as dated items
Below the nine fields, one line per obligation your side owes: what, who here, by when, and how often if it repeats. Obligations the other side owes are worth listing too when you are relying on them, such as a service report or a maintenance window. Keep them as items rather than prose, because prose cannot be sorted and an obligation you cannot sort is one you will not chase.
The fields to leave out
Anything only one agreement has. Governing law, unless you operate across jurisdictions where it varies and matters. Full clause text, because that is what the attached document is for. Commentary about the relationship, which ages badly and is read as fact later. A summary that is longer than a screen has stopped being a summary and become a second contract.
Keeping it true after a change
The summary is wrong the moment an amendment, addendum or extension is signed unless it is updated then. Make updating it part of filing the change rather than a separate task, and note in one line what each change did. A summary that is right about the original and silent about three amendments is the most misleading document in the whole record.
